Provide a copy of certificate of registration for VAT | |
| Provide a current copy of CITB levy and confirmation of payment | |
| Provide a copy of company accounts for the last two financial years | |

Provide copies of your current Employers Liability and Public Liability insurance certificates | |
| Demonstrate 75% of your working operatives are directly employed (by providing a copy of previous weeks payroll details)l | |
| Demonstrate 75% CISRS carded operatives via the Scaffolders Registration and Training Record sheet | |
| Provide a current signed Company Health and Safety Policy Statement | |
| Provide a copy of Safety Harness issue and inspection document | |
| Provide a copy of full PPE issue document | |
